Chances of Pregnancy with Endometrial Polyps

Endometrial or uterine polyps are very common in women within the reproductive age range and can affect the chances of pregnancy. They are one of the most common causes of abnormal uterine bleeding. The polyps form on the endometrium, which is the interior lining of the uterus.

Endometrial polyps are generally benign, although it is possible for them to be malignant or premalignant. As a precaution, when an endometrial polyp is removed, it is always sent to a laboratory to be examined for any sign of cancer.

Uterine bleeding is a symptom of the polyps, but endometrial polyps can exist within the uterus and not cause bleeding. They are commonly found only when a patient undergoes fertility testing. One of the common tests during infertility testing is an x-ray of the uterus and fallopian tubes, called a Hysterosalpingogram (HSG).

Performing a Hysterosalpingogram (HSG)

During a Hysterosalpingogram, the patient empties her bladder, removes her clothing below the waist and lies on her back on an examination table. Ibuprofen may be taken a few hours before the procedure to lessen discomfort.

A speculum is inserted into the vagina to reveal the cervix. A thin catheter is inserted through the cervix into the uterus, and a small amount of dye is then injected through the catheter into the uterus and fallopian tubes.

As the dye fills the uterus and fallopian tubes, a series of X-rays are taken, and then the speculum and catheter are removed. Endometrial polyps are often found when undergoing this type of x-ray.

What Are the Chances of Pregnancy After Polyp Removal?

A study of the association of endometrial polyps with fertility determined that the chance of pregnancy in women with endometrial polyps was only 28%. However, the study found that having the polyps removed increased the pregnancy rate to 63%.

Another study found that the polyp’s location in the uterus also had a bearing on the pregnancy rates. Polyps removed from the area around the opening of the fallopian tubes into the uterus were found to provide a higher chance of pregnancy than those removed from other areas of the uterus. This helps doctors estimate the chances of pregnancy after polyp removal and suggests that polyps in this area create more hindrances to pregnancy than those in other areas.

Endometrial Polyp Surgery

Undergoing removal of endometrial polyps may be the best course of action for those trying to conceive. For this, the patient can be under local or general anesthesia, depending on the size and number of the polyps. The procedure usually takes less than one hour and is typically performed on an outpatient basis, with the patient cautioned against driving home afterwards.

The patient lies on an examination table, and a speculum is inserted into the vagina to dilate the cervix and allow access to the uterus. The most common way to remove the polyps surgically is by undergoing a Transcervical Resection of Fibroids (TCRF). The procedure most often uses a small wire loop that twists the polyp off the wall of the uterus. The base of the polyp is then cauterized to stop any bleeding.

The surgeon is able to see the procedure through an instrument resembling a small telescope called a hysteroscope.
